SUBJECT: Indoor Air Quality (IAQ) Assessment
Portable 701VE and 702VE

On September 15, 2006 I conducted an assessment of Portable 701VE and 702VE at Hollywood Hills
Elementary School. This evaluation included observations of the flooring system, ceiling tiles, false
ceiling plenum, environmental surfaces, interior and exterior walls, and the accessible ventilation
equipment. Additionally, environmental parameter measurements were taken to include
temperature, relative humidity, and carbon dioxide. The detailed findings, along with the
recommended corrective action can be found on the attached IAQ Assessment Worksheets.

The IAQ assessment did identify one or more existing conditions impacting IAQ and has generated
appropriate work orders to correct deficiencies in systems and maintenance that could contribute to
decreased indoor air quality. At the time of the assessment, these concerns were not an immediate
health or safety concern to building occupants. However, due to individual sensitivities and
predisposing health factors, it is possible that some building occupants may elicit a health response to
agents and / or conditions identified during the evaluation. Therefore, to further improve IAQ,
prevent development of future IAQ-related problems, and to reduce the potential for IAQ-related
complaints by building occupants, the IAQ Assessment Team recommends appropriate follow up of
each item identified and listed in the attached evaluation.

Observations
Findings:
- Dust and debris on HVAC supply and return grills
- Temperature was elevated at the time of the assessment. Only East A/C unit was running. Fresh air vent was closed and was opened at the
time of the assessment.
- Dust build up on environmental surfaces
- Dust and debris on HVAC coils - East unit
- Elevated moisture content in walls under all windows and all of West wall
- Exterior - caulking missing around South door and around windows on North side
- Exterior - Fungus growing on West wall and T-1-11 is soft to the touch
- Exterior - Possible birds nesting above ceiling as evidenced by soffit screen in Norhwest corner of portable

Recommendations:
Site Based Maintenance:
- Clean HVAC supply and return grills with Wexcide disinfecrtant solution
- Ensure that both HVAC units are running and set to fresh air
- Thoroughly clean environmental surfaces
- Continue to monitor this location for any signs of microbial growth as well as dust and debris accumulation and clean as appropriate
Physical Plant Operations:
- Clean HVAC coils - East unit
- Evaluate cause of water intrusion and repair. Repair/replace wall material as appropriate (see above).
- Caulk around South door and around indows on North side
- Replace damaged T-1-11 on exterior of portable
- Seal birds out of the building - Northwest corner of portable

Observations
Findings:

- Dust and debris on HVAC coils - both units. Coils frozen on West unit.
- Elevated moisture content in East and West wall and South wall from restroom door to portable door
- Exterior - T-1-11 on West side is severely water damaged
- Exterior - T-1-11 patch on South side is allowing water into the building - entire length of the wall
- Exterior - caulking missing around windows and doors

Recommendations:

Site Based Maintenance:
- Clean HVAC supply and return grills with Wexcide disinfecrtant solution
- Remove and replace A/C filters
- Thoroughly clean environmental surfaces
- Continue to monitor this location for any signs of microbial growth as well as dust and debris accumulation and clean as appropriate
